The Vitality Project is a process whereupon, an individually-designed health and wellness program is provided to the client concurrently with personal coaching, peer support, on-going education, assessment and skill transference.
The Vitality Project does not simply involve a process of disease identification and treatment. More specifically, The Vitality Project gives the client the tools to be successful at eventually improving their own current level of health and well-being while subsequently enhancing their own personal sense of vitality through time.
What are the steps necessary for a client to complete The Vitality Project process?
Step One: Data Collection
Each new client who becomes a member of The Vitality Project undergoes a comprehensive health assessment that inevitably involves the exchange of information with the client which is kept strictly confidential.
Step Two: Data Interpretation
After the data from the client is received, it is analyzed. Following this procedure and depending upon individual circumstance, additional client information may be requested. Upon completion of step two, a client needs analysis is completed to determine the key components upon which to base the program design. Subsequently, the most important physiological end-points can be calculated and in collaboration with the client’s own preferences and objectives, a comprehensive health and wellness strategy can be designed.
Step Three: Program Development and Delivery
The program will be designed to provide the necessary services to the client with the intent of measuring specific end-points and outcomes which are essential to evaluate and assess the effectiveness of any health care program. This program will review the key deliverables to the client and provide the related rationale. The Vitality Project protocols of health management utilize nutrition, exercise and lifestyle modification concepts to deliver a wellness-based, customized, client-centred program of care.
Step Four: Program Support
In combination with a comprehensive program provided to The Vitality Project client, individualized, coaching strategies implementing e-mail, Skype and at times, telephone consultations can be arranged. In addition to this service, peer-support is also provided and encouraged by the utilization of a web-based client portal which may be accessed through the client’s area on The Vitality Project website. Various links to social media tools are also available to allow the full participation of all The Vitality Project’s client’s thus encouraging open communication and participation between the client community. Additionally, personal consultations for The Vitality Project client may also be arranged.
Other support tools such as client resources, questionnaires, an active blog, webinars, e-books, research archive and various u-tube presentations are also available in the client area of the website.
Step Five: Client Assessment
In order for any health delivery system to be deemed affective, an evaluation process contained within the process has to be regularly implemented so that the client’s changing health requirements are accounted for. In adherence to the aforementioned, the client will have continued access to individualized coaching, program assessment and re-design with their continued monthly membership.
The Vitality Project provides a comprehensive report to the client following the sixth and twelfth month of program involvement. This report will demonstrate the positive changes that the client has made in their own health status and also highlight some of the potential challenges that the client may also need to consider for the future.
